Treatment of invasively ventilated adults with Early Activity and Mobilisation

PICO Question

  • Population: Ventilated patients within 48 hours of initiation of mechanical ventilation

  • Interventions: Clonidine ± opioid analgesic or Dexmedetomidine ± opioid analgesic

  • Comparison: Usual care sedation with propofol ± opioid analgesic

  • Outcome: Time from randomisation to successful extubation

Inclusion criteria

Exclusion criteria

  • Aged >18y

  • Ventilated, and expected to remain so >48h

  • Sufficient cardiovascular stability

  • Sufficient respiratory stability

  • Dependent for ADLS prior to current ICU admission

  • Documented cognitive impairment

  • Acute primary brain pathology

  • Spinal cord injury/neuromuscular (excluding ICU acquired weakness)

  • Requires bed rest / bilateral non-weight bearing for lower limbs

  • Life expectancy <180 days

  • Death inevitable/limitations/withdrawal

  • Unable to communicate in English

  • ICU admission ≥72h after filling inclusion criteria
